Outsourced home health ICD-10 coding for agencies nationwide, done directly in your EHR. Our physician-led team codes from a full review of the OASIS and clinical record — OASIS review included at no separate charge — and returns primary and secondary diagnosis coding, sequencing, and flagged documentation discrepancies within 2 business days at $50 per case. Orders and status live in Boltflo; there is nothing to export or upload. We work with home health agencies in all 50 states; your location does not change the service or the turnaround.

Offer terms: New agencies only; one offer per agency. Three cases total, choosing coding only, 485 only, or the full bundle. Standard onboarding, service agreement, secure access, turnaround times, and hold policies apply. No automatic paid enrollment — regular pricing is disclosed before the agency authorizes additional paid work. Free cases go through our normal workflow. Your assigned billing team records your first three eligible completed work orders and applies invoice credits equal to their actual charges — $70 per bundle, $50 per coding-only case, $25 per 485-only case, or the corresponding total for a mix. If fewer than three are completed by your first invoice, the remaining free cases carry over to later invoices; each eligible case is credited once.
What We Review Before Coding
Before completing ICD-10 coding, Codexpo fully reviews the agency-submitted records, OASIS information, diagnosis support, medication details, and related documentation to identify missing or inconsistent information that may affect accurate coding. This OASIS and record review is included in every order at no separate charge.
Clinical Documentation Referral records, hospital notes, physician documentation, and supporting clinical records are reviewed before ICD-10 coding is completed.
OASIS Information OASIS assessment data is reviewed alongside the clinical record to support diagnosis selection and check for consistency with the documented patient condition.
Diagnosis Support Primary and secondary diagnoses are reviewed for documentation support, sequencing, and alignment with the home health plan of care.
Documentation Gaps Missing, unclear, or inconsistent information is flagged so your team can follow up before coding is finalized and submitted for review.
Outsourced Home Health Coding Without Changing Your EHR
Codexpo works as your agency’s outsourced coding team inside the EHR you already use — WellSky (Kinnser), Axxess, Homecare Homebase, Synergy, MatrixCare, Alora, KanTime, Netsmart and AlayaCare — with the access you provide. You submit a work order in Boltflo for each patient’s SOC, recertification or ROC; we review the OASIS and records, code the diagnoses in your EHR, and return the case with sequencing notes and any discrepancies flagged for your clinician, who keeps final approval.
ICD-10 Coding is $50 per case, returned within 2 business days. Need the plan of care drafted too? The Coding + 485 Bundle is $70 per case within 5–6 business days, and 485 Plan of Care Draft & Review on its own is $25. OASIS review is included with all three. How It Works
A straightforward, physician-led workflow designed to fit into your agency’s existing process.
Place Your Order in Boltflo
Submit a Coding only order in Boltflo, our secure case portal. Codexpo works directly in your EHR using the access your agency provides, so you do not need to export or upload records our assigned team can already access. Your turnaround clock starts when you submit your order.
Physician-Led Review & Coding
Our physician-led team fully reviews the OASIS and referral documentation in your EHR, then completes ICD-10 primary and secondary diagnosis coding and sequencing aligned with the documented patient condition and the home health plan of care.
Gaps & Inconsistencies Flagged
Missing, unclear, or conflicting information is flagged for your clarification. Your turnaround clock starts when you submit your order. If required records are missing, we place the case on hold and identify what is needed. Time on hold does not count toward turnaround, and the due date is adjusted accordingly. A reply does not resume processing on its own: our staff review the reply and clear the hold once the requested information is sufficient. If a reply is incomplete, the case stays on hold and we follow up with questions in the same case thread.
Coded Record Returned
The completed coding is returned in the Boltflo case within 2 business days for your team’s review. Codexpo does not finalize or sign anything in your record; final review and approval remain with your agency.
What a Returned Coding Case Looks Like
Illustrative example: fictional patient and findings. This sample shows the kind of information a returned coding case contains. It is not an actual Codexpo return or a real patient record, and the layout of your returns in Boltflo may differ.
Order: Coding only, start of care. Status: returned for agency review.
- Diagnosis coding: primary and secondary ICD-10 diagnoses coded and sequenced in your EHR.
- Sequencing note: a short note explaining the order of the diagnoses.
- Flag 1, diagnosis support: the referral lists a condition that does not appear in the physician documentation. Please clarify before it is coded.
- Flag 2, OASIS consistency: the OASIS describes the patient as walking independently, while the therapy evaluation documents a walker and standby assistance. Please review.
- Flag 3, medications: the hospital discharge medication list and the medication profile in the EHR do not match. Please reconcile.
Your agency reviews the coding and every flag, makes any changes, and keeps final approval.

Frequently Asked Questions
What documentation do you need to begin coding?
Typically referral records, physician documentation, OASIS information, and any supporting clinical or medication records. Our assigned team reads these directly in your EHR using the access you provide, so nothing needs to be exported or uploaded; anything missing or inaccessible is requested through the case. The more complete the documentation, the more accurate and consistent the coding can be.
Do you code from the OASIS assessment?
OASIS information is reviewed alongside the clinical record to support diagnosis selection and check for consistency, but coding is based on the documentation as a whole rather than the OASIS alone. Our article on how OASIS accuracy and ICD-10 coding work together under PDGM covers this in more detail.
Who reviews the coding?
Coding is completed under physician-led oversight. Diagnoses are reviewed for documentation support, sequencing, and alignment with the home health plan of care before the record is returned to your team.
How long does turnaround take?
ICD-10 coding is returned within 2 business days. A 485-only order is returned within 3–4 business days, and a Coding + 485 Bundle within 5–6 business days in total, because the 485 work begins once coding is finished. Your turnaround clock starts when you submit your order in Boltflo.
Time-sensitive cases are handled under Codexpo’s internal priority policy: cases approaching our internal completion target are prioritized in our work queues. Priority handling does not shorten the standard turnaround commitment and does not replace or extend your agency’s own regulatory deadlines.
How do we send you records?
Submit the order in Boltflo, our secure case portal: log in, choose Coding only, 485 only, or Bundle, and enter the patient. Codexpo works directly in your agency’s EHR using the access you provide, so you do not need to export or upload records our assigned team can already access. You can then monitor the case status, respond to any missing-record request, and receive the completed work in the same case. EHR access is arranged with our team during onboarding — never through the public contact form. OASIS review is included with every option.
What happens if a chart is missing something?
Your turnaround clock starts when you submit your order. If required records are missing, we place the case on hold and identify what is needed. Time on hold does not count toward turnaround, and the due date is adjusted accordingly. A reply does not resume processing on its own: our staff review the reply and clear the hold once the requested information is sufficient. If a reply is incomplete, the case stays on hold and we follow up with questions in the same case thread.
How do corrections work?
Request a correction from the case in Boltflo — coding, 485, or both — and track it to resolution in the same case.
How are we billed?
Monthly, with an itemized invoice that lists every case. One case is one work order for one patient’s start of care (SOC), recertification, or resumption of care (ROC), billed at the rate of the service you select. A Coding + 485 Bundle counts as one case. Corrections, holds, and messages do not create additional cases. See home health coding pricing for each option.
Which EHRs do you work in, and do we have to export records?
We code directly in your agency’s EHR using the access you provide — WellSky (Kinnser), Axxess, Homecare Homebase, Synergy, MatrixCare, Alora, KanTime, Netsmart and AlayaCare — so there is nothing to export or upload. Orders, status, missing-record requests and the finished coding live in Boltflo, our secure case portal. See the EHR systems we work in and how it works.
