485 Plan of Care Draft & Review Services

Outsourced 485 Plan of Care Drafting for Home Health Agencies

485 Plan of Care draft and review for home health agencies nationwide, prepared by a physician-led team directly in your EHR. Every order starts with a full OASIS and record review (included at no separate charge); we then draft and review the plan of care and return it in Boltflo for your agency’s review and approval — within 3–4 business days for a 485-only order at $25 per case, or within 5–6 business days in total as part of the $70 Coding + 485 Bundle. Agencies in all 50 states order 485 drafting the same way: a work order in Boltflo, the draft prepared in your EHR, and final review and approval with your agency.

Offer terms: New agencies only; one offer per agency. Three cases total, choosing coding only, 485 only, or the full bundle. Standard onboarding, service agreement, secure access, turnaround times, and hold policies apply. No automatic paid enrollment — regular pricing is disclosed before the agency authorizes additional paid work. Free cases go through our normal workflow. Your assigned billing team records your first three eligible completed work orders and applies invoice credits equal to their actual charges — $70 per bundle, $50 per coding-only case, $25 per 485-only case, or the corresponding total for a mix. If fewer than three are completed by your first invoice, the remaining free cases carry over to later invoices; each eligible case is credited once.

What We Review on the 485

Before finalizing any 485 draft, Codexpo fully reviews the agency-submitted OASIS and records to confirm the Plan of Care is consistent, complete, and supported by the documentation provided. This OASIS and record review is included in every order at no separate charge.

Orders & Interventions

Physician orders, frequencies, and interventions are reviewed for clarity and consistency with the documented patient condition.

Diagnosis Alignment

Primary and secondary diagnoses on the 485 are checked against the OASIS and clinical record for sequencing and support.

Goals & Visit Frequencies

Care goals, disciplines, and visit frequencies are reviewed for completeness and internal consistency.

Documentation Gaps

Missing, unclear, or conflicting details are flagged so your team can follow up before the form is finalized and signed.

Home Health 485 Review, Done in Your EHR

You submit a work order in Boltflo for the patient’s SOC, recertification or ROC; we work inside the EHR your agency already uses — WellSky (Kinnser), Axxess, Homecare Homebase, Synergy, MatrixCare, Alora, KanTime, Netsmart and AlayaCare — with the access you provide, so there is nothing to export or upload. The 485 draft and review findings come back in the case, and Codexpo does not sign or finalize the 485; final review and approval stay with your agency.

485 Plan of Care Draft & Review is $25 per case within 3–4 business days. If you also need the diagnoses coded, the Coding + 485 Bundle is $70 per case within 5–6 business days in total (the 485 work begins once ICD-10 coding is finished). OASIS review is included with every option. See how it works and the EHR systems we work in. For a step-by-step walkthrough, read how coding and 485 review work inside your agency’s EHR.

How It Works

A straightforward, physician-led workflow designed to fit into your agency’s existing process.

1

Place Your Order in Boltflo

Submit a 485 only order in Boltflo, our secure case portal — or a Coding + 485 Bundle if you also need coding. Codexpo works directly in your EHR using the access your agency provides, so you do not need to export or upload the OASIS, orders, or records our assigned team can already access. Your turnaround clock starts when you submit your order.

2

Physician-Led 485 Fill-In & Review

Our physician-led team fully reviews the OASIS and records in your EHR, then prepares the 485 fill-in draft and reviews it against the documentation.

3

Gaps & Inconsistencies Flagged

Missing, unclear, or conflicting information is flagged for your clarification. Your turnaround clock starts when you submit your order. If required records are missing, we place the case on hold and identify what is needed. Time on hold does not count toward turnaround, and the due date is adjusted accordingly. A reply does not resume processing on its own: our staff review the reply and clear the hold once the requested information is sufficient. If a reply is incomplete, the case stays on hold and we follow up with questions in the same case thread.

4

Draft Returned for Approval

The completed 485 draft is returned in the Boltflo case — within 3–4 business days for a 485-only order, or 5–6 business days in total for a Coding + 485 Bundle — for your agency’s review and approval. Codexpo does not sign or finalize the 485.

What a Returned 485 Draft Looks Like

Illustrative example: fictional patient and findings. This sample shows the kind of information that comes back with a 485 draft. It is not an actual Codexpo return or a real patient record, and the layout of your returns in Boltflo may differ.

Order: 485 only, recertification. Status: draft returned for clinician approval.

  • Draft: the 485 Plan of Care prepared in your EHR from the OASIS, orders, and clinical record, ready for your clinician to review.
  • Flag 1, orders and frequencies: the visit frequency in the physician order differs from the frequency documented in the clinical notes. Please confirm which is current.
  • Flag 2, diagnosis alignment: a diagnosis on the draft is sequenced differently from the OASIS. Please review before finalizing.
  • Flag 3, medications: a medication in the hospital discharge summary is missing from the medication profile. Please reconcile.
  • Holds: none.

We do not sign or finalize the 485. Your agency reviews the draft and every flag and keeps final approval.

Frequently Asked Questions

Do you sign or finalize the 485?

No. We prepare the fill-in draft and review it; final review, approval and signature always remain with your agency.

What do you need to prepare a 485 draft?

Typically the OASIS, physician orders, referral and clinical documentation, and any supporting records. Our assigned team reads these directly in your EHR using the access you provide, so nothing needs to be exported or uploaded; anything missing or inaccessible is requested through the case. More complete documentation supports a more consistent draft.

How is the 485 reviewed for accuracy?

Under physician-led oversight, the draft is checked against the OASIS and clinical record for diagnosis support, order consistency, and documentation alignment. Our article explains why accurate ICD-10 coding and 485 review matter for home health agencies.

How long does a 485 draft take?

A 485-only order (full OASIS and record review, then the plan-of-care draft and review) is returned within 3–4 business days. A Coding + 485 Bundle is returned within 5–6 business days in total, because the 485 work begins once coding is finished; coding alone is returned within 2 business days. Your turnaround clock starts when you submit your order in Boltflo.

Time-sensitive cases are handled under Codexpo’s internal priority policy: cases approaching our internal completion target are prioritized in our work queues. Priority handling does not shorten the standard turnaround commitment and does not replace or extend your agency’s own regulatory deadlines.

How do we send you the records?

Submit the order in Boltflo, our secure case portal: log in, choose 485 only (or Bundle if you also need coding), and enter the patient. Codexpo works directly in your agency’s EHR using the access you provide, so you do not need to export or upload the OASIS, orders, or supporting documentation our assigned team can already access. You can then monitor the case status, respond to any missing-record request, and receive the completed draft in the same case. EHR access is arranged with our team during onboarding — never through the public contact form.

What if something is missing or an order changes?

Your turnaround clock starts when you submit your order. If required records are missing, we place the case on hold and identify what is needed. Time on hold does not count toward turnaround, and the due date is adjusted accordingly. A reply does not resume processing on its own: our staff review the reply and clear the hold once the requested information is sufficient. If a reply is incomplete, the case stays on hold and we follow up with questions in the same case thread. When a physician order adds a discipline, record it in the case and we adjust the draft.

How do corrections work after the draft is returned?

Request a correction from the case in Boltflo — 485, coding, or both — and track it to resolution in the same case.

How are we billed?

Monthly, with an itemized invoice that lists every case. One case is one work order for one patient’s start of care (SOC), recertification, or resumption of care (ROC), billed at the rate of the service you select. A Coding + 485 Bundle counts as one case. Corrections, holds, and messages do not create additional cases. See home health coding pricing for each option.

Which EHRs do you work in, and do we have to export records?

We prepare and review the 485 directly in your agency’s EHR using the access you provide — WellSky (Kinnser), Axxess, Homecare Homebase, Synergy, MatrixCare, Alora, KanTime, Netsmart and AlayaCare — so there is nothing to export or upload. Orders, status, missing-record requests and the finished draft live in Boltflo, our secure case portal. See the EHR systems we work in and how it works.
