Our Quality & Review Process

Our Quality & Review Process

Prospective agencies ask us two fair questions: how much work do you actually handle, and who checks it before it comes back to us? This page answers both.

The scale of our work

Codexpo currently serves 1,021 active home health agencies and handles approximately 15,000 cases each month — about 14,600 coding + 485 bundles, plus another 400–500 coding-only or 485-only cases. Each coding + 485 bundle counts as one case, even though it includes both services and the included OASIS review.

Agency count and approximate monthly volume reported by Codexpo management, November 2026. These are operational figures, not an audited monthly average.

How clinical review works

Every order starts with the record, not the code list. We review the available clinical documentation and OASIS information together, evaluate whether the documentation supports each diagnosis we select, and sequence diagnoses to reflect the documented patient condition. When something is missing, unclear, or inconsistent, we flag it for clarification instead of guessing.

Final clinical review, approval, and signature always stay with your agency’s team. Codexpo does not sign or finalize records.

Who oversees the work

Dr. Mher Khachatryan has worked in coding since 2010. He leads Codexpo’s approach to clinical review and diagnosis sequencing and guides the training of the professionals selected to review and approve coding.

Coding is reviewed and approved by Dr. Mher or trained professionals personally selected by him and trained under his guidance.

Corrections and questions

If anything in returned work needs a second look, request a correction from the case in Boltflo — coding, 485, or both — and track it to resolution in the same case. Corrections are reviewed under the same oversight as the original work. When a physician order adds a discipline, record it in the case and we adjust the draft.

Questions about a specific code or sequencing choice go through the same case thread, so the discussion stays attached to the record it concerns.
